BBS Front Sheet of Log Book Posted: Wed, January 16, 2008

Many of you will have started work by this stage and it is therefore time for you to let us have the completed front sheet of your log-books.  It is essential that we obtain this information since it gives the name and address of the employer, name of your supervisor and contact e-mail addresses and telephone numbers.  The template for the log book header page is on the placement website. 

Please note that this sheet needs to be signed by your work supervisor and then posted or faxed (on 051 302 688) to Annette Toms at the School of Business, WIT.  Many thanks to those of you who have already sent on the front page of your Log Book - no need to resend it.

As regards the rest of the log book, there are notes on the web-site version, and a completed example is also available from the website .  You are advised to think about your log-books sooner rather than later and, if you have any questions to contact your mentor or one of the work placement team.
